How robotic-assisted surgery works

Robotic-assisted knee replacement is the same operation as a conventional total knee replacement, performed with robotic-arm guidance for the bone cuts. Before surgery, a CT scan builds a three-dimensional model of your knee. The surgeon plans the implant position and bone cuts on that model, and during the operation the robotic arm guides the cuts to the plan.

The surgeon performs the operation throughout. The robot does not operate independently; it constrains the cuts to the pre-operative plan.

Both of our partner hospitals offering the robotic option use the Stryker MAKO platform. KDMS publishes that its packages use Stryker implant components from the United States.

What the evidence actually shows

Most sites selling robotic surgery say it is simply better. The trial evidence says something more specific, and this site's rule is to report it as found.

A 2025 analysis of 21 randomised trials covering 2,692 patients found robotic-assisted knee replacement achieved better mechanical alignment accuracy than conventional surgery, but did not show superior short to medium-term functional outcomes, and took longer in theatre. An earlier meta-analysis of randomised trials (2024) found the same pattern: superior alignment, similar clinical and functional outcomes and complication rates.

On implant survival, a Michigan registry analysis found similar five-year revision rates for robotic and conventional knees: cemented 3.9% versus 3.5%, cementless 1.8% versus 2.8%. Long-term registry data on robotic surgery is still accumulating, and long-term cost-effectiveness is not established.

In one sentence: better alignment, no proven functional advantage yet, longer in theatre, comparable revision rates so far. That is the honest position.

Who benefits most

Whether robotic assistance would change anything in your case is a judgement your surgeon makes from your imaging and anatomy, not a rule that applies to everyone. At your assessment, ask two questions: would robotic guidance change anything for my anatomy specifically, and what is your own experience with both approaches. A surgeon who answers those directly is telling you something useful.

Who is this not right for? If the roughly 200,000 THB premium stretches your budget, the evidence gives no reason to believe you are compromising your outcome by choosing the standard operation. A conventional total knee replacement is the most studied joint operation in existence.

The packages

Two partner hospitals offer robotic-assisted knee replacement, both on the MAKO platform, both with the implant included.

KDMS Hospital is Thailand's first hospital dedicated exclusively to bone and joint conditions, and its published package terms cover 5 days and 4 nights in a standard inpatient room for one knee, with nursing, hospital services, meals and equipment charges included.

Bumrungrad International Hospital, JCI-accredited since 2002, offers the robotic option at the higher price point with the largest international patient infrastructure of the three partners. It does not publish its package contents; your written quote states the inpatient stay and inclusions.

Is it worth the extra cost?

The robotic packages cost roughly 200,000 THB more than the standard operation at the same hospitals (510,000 THB versus 283,000 THB at KDMS). What that buys, on current evidence, is more precise implant alignment. What it has not yet been shown to buy is better function, faster recovery or a longer-lasting implant.

Some patients value the precision and the pre-operative CT planning, and choose robotic on that basis. Others take the trial evidence at face value and choose standard. Both are reasonable choices, and we will not pretend the evidence settles it. The full comparison is in the complete guide and the cost breakdown on the cost page.

Recovery differences

Recovery follows the same path as a conventional knee replacement, and the trial evidence found no consistent difference in short to medium-term function. Walking training starts the day after surgery, NHS guidance puts unaided walking at around six weeks and driving at no earlier than six weeks, and full recovery takes several months or longer. Plan your stay in Bangkok around your surgeon's clearance, not the package.

FAQ

What is MAKO robotic knee replacement?

A robotic-assisted technique using the Stryker MAKO platform. A CT scan builds a three-dimensional model of your knee before surgery, the surgeon plans the bone cuts on that model, and a robotic arm guides the cuts to that plan during the operation. The surgeon performs the operation; the robot does not operate independently.

Is robotic knee replacement better than conventional?

The evidence supports better precision, not better outcomes, at least so far. A 2025 analysis of 21 randomised trials covering 2,692 patients found robotic-assisted surgery achieved better mechanical alignment than conventional surgery, but did not show superior short to medium-term functional outcomes, and took longer in theatre.

Do robotic knees last longer?

There is no evidence yet that they do. A Michigan registry analysis found similar five-year revision rates for robotic and conventional knees, 3.9% versus 3.5% for cemented and 1.8% versus 2.8% for cementless. Long-term registry data on robotic surgery is still accumulating.

Is robotic worth the extra cost?

At our partner hospitals the robotic option costs roughly 200,000 THB more than standard. What that buys, on current evidence, is more precise implant alignment. What it has not yet been shown to buy is better function or a longer-lasting implant, and long-term cost-effectiveness is not established. Some patients value the precision; it is a reasonable choice either way.

Who benefits most from robotic surgery?

That is a judgement your surgeon makes from your imaging and anatomy rather than a rule. Ask specifically whether robotic assistance would change anything in your case, and what the surgeon's own experience is with both approaches.
